Recieved Doctors Report for an home visit in relation to an Adjourned Appeal.

However, on the Report (DLA Documents) the GP has written some answers down but did NOT ask the questions relating to his answers !

How can this be resolved ?

One example to a question is: Does the clients condition fluctuate/change. Answer is: Does not change

Problem being, above question was not asked. The truth about fluctuate/change is: Can be in bed for 48hours due to sever pain in legs.

Also the GP was 30mins late, resulting the visit taking 30mins and NOT 60mins as stated by GP on report.
